RN Hospice PRN – Home Care

About the role

The RN Hospice PRN provides professional, empathetic nursing care to patients in their homes across Greater Cleveland. This flexible, as‑needed position supports patients, families, and the care team while helping prevent readmissions.

Key responsibilities

• Deliver direct nursing care in accordance with organizational policies and protocols.
• Perform comprehensive admission assessments, reassessments, and ongoing patient monitoring.
• Develop, implement, and monitor individualized patient care plans.
• Supervise LPNs, Home Care Aides, and family caregivers.
• Assess and manage patient pain using appropriate scales.
• Document patient conditions, interventions, and outcomes accurately and timely.
• Coordinate multidisciplinary care and escalate changes in condition to physicians.

Required profile

• Graduate of an accredited nursing program with current RN licensure.
• Basic Life Support (BLS) certification through AHA or Red Cross.
• At least one year of clinical experience in an acute medical/surgical or home‑care setting.
• Valid Ohio driver’s license and proof of automobile insurance.
