Advanced Practice Provider – Mobile Wound Ostomy & Continence Care (PRN)

About the role

The Advanced Practice Provider will deliver mobile wound, ostomy and continence care to patients in their homes on an as‑needed (PRN) basis. This role supports the Center for Connected Care’s mission to provide high‑quality, patient‑centered care across multiple regions. The provider will work collaboratively with multidisciplinary teams to ensure optimal outcomes.

Key responsibilities

• Obtain comprehensive medical histories, perform physical examinations, and order appropriate laboratory, radiology, or special tests.
• Interpret diagnostic results, develop working diagnoses, and create treatment plans in collaboration with the health‑care team.
• Prescribe and monitor medications within state licensing and Cleveland Clinic policies.
• Educate patients and families about disease processes, treatments, medication side effects, and self‑care techniques.
• Participate in multidisciplinary rounds, referrals, discharge planning, and quality monitoring activities.
• Serve as a clinical preceptor for students and assist with onboarding of new caregivers.

Required profile

• Graduate of an accredited APRN program (MSN required) or an A.R.C. approved Physician Assistant program (Master’s degree).
• Current licensure as a Nurse Practitioner, Physician Assistant, or Clinical Nurse Specialist in the applicable state(s).
• National certification by the Wound, Ostomy and Continence Nursing Certification Board.
• Basic Life Support (BLS) certification; ACLS, PALS, NRP, or ATLS as required by the work setting.
• Credentialed and privileged by the Medical Executive Committee and the governing body of the applicable hospitals.
