Occupational Therapist – Home Care

About the role

The Occupational Therapist will provide in‑home patient assessment and treatment for a diverse caseload, supporting individuals with orthopedic, cardiovascular and neurological diagnoses. You will work independently to schedule visits, coordinate travel routes and collaborate with interdisciplinary teams to ensure optimal patient outcomes.

Key responsibilities

• Perform initial and ongoing assessments of patients’ conditions and develop individualized plans of care.
• Deliver therapy interventions using standard occupational therapy techniques appropriate to age and condition.
• Coordinate daily schedules with patients, arrange visit times and travel routes, and attend team care conferences.
• Supervise occupational therapy assistants, students and other support personnel.
• Maintain complete and up‑to‑date documentation of patient progress and interventions.

Required profile

• Bachelor’s degree from an AOTA‑accredited Occupational Therapy program.
• Current state licensure as an Occupational Therapist.
• Current Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and automobile insurance meeting policy requirements.

Required skills

• Ability to walk without assistive device, maintain balance and lift/reach to support patients during treatment.
• Manual dexterity and tactile sense to palpate muscle tension and monitor pulse.
• Clear verbal communication for instructing patients and families; adequate hearing for conversational level.
