RN Hospice Coordinator – West and South Region

About the role

The RN Hospice Coordinator supports patients transitioning from hospital to home or facility across the West and South regions. Working from the Business Operations Center, you will ensure seamless hospice admission, discharge planning, and continuity of care for patients and families.

Key responsibilities

• Process inpatient hospice referrals, coordinate discharge plans, and arrange medical supplies and DME.
• Educate patients, families, and referral sources on hospice benefits and eligibility.
• Conduct admission assessments, document in the EMR, and collaborate with interdisciplinary teams.
• Perform daily safety, pain, and symptom assessments and update care plans accordingly.

Required profile

• Graduate of an accredited nursing program with current RN licensure.
• At least one year of recent clinical experience in an acute med/surg or critical‑care setting.
• Basic Life Support (BLS) certification and knowledge of JCAHO and Medicare regulations.
• Ability to work irregular hours, travel to hospitals and community sites, and handle physical demands of the role.

Required skills

• Basic Life Support (BLS) certification.
• Proficiency with electronic medical record (EMR) systems.
• Knowledge of hospice admission criteria and medication dosing.
