RN Homecare PRN – Summit/Medina Counties

About the role

The Cleveland Clinic Health System is seeking a PRN Registered Nurse to provide high‑quality home health care across Summit and Medina counties. You will deliver assessments, complex nursing treatments, and patient education while coordinating care remotely.

Key responsibilities

• Provide direct nursing care and complete admission assessments and reassessments in patients' homes.
• Assess safety risks, abuse, and neglect, and serve as primary coordinator for multidisciplinary care.
• Document visits, make follow‑up calls, attend meetings, and arrange patient visit schedules.
• Manage patients' pain, educate families, and work to prevent readmissions to acute care.

Required profile

• Graduate from an accredited nursing program with current RN licensure.
• At least one year of recent RN experience in acute medical/surgical or home‑care settings.
• Basic Life Support (BLS) certification and a valid Ohio driver’s license.
• Proof of automobile insurance coverage as required by policy.
