RN Homecare PRN – Per Diem Nursing Position

About the role

The Cleveland Clinic Health System seeks a Registered Nurse for homecare on a PRN (as‑needed) basis. The RN will deliver direct nursing care, coordinate multidisciplinary services, and supervise support staff while ensuring compliance with organizational policies.

Key responsibilities

• Perform admission assessments, reassessments, and ongoing patient evaluations within required time frames.
• Develop, implement, and monitor individualized patient care plans, including pain management and safety risk assessments.
• Coordinate care among LPNs, home‑care aides, family caregivers, and other disciplines.
• Document patient conditions, interventions, and outcomes accurately and promptly.
• Escalate changes in patient status to the attending physician as needed.

Required profile

• Graduate of an accredited nursing program; BSN preferred.
• Current Ohio RN license and Basic Life Support certification.
• Minimum one year of acute medical‑surgical nursing experience; at least six months in home‑care nursing.
• Valid Ohio driver’s license and proof of automobile insurance as per policy.

Required skills

• Basic Life Support (BLS) certification.
• Clinical assessment and documentation skills.
