RN Homecare – Eastern Cuyahoga & Lake (PRN)

About the role

The RN Homecare position supports patients in Eastern Cuyahoga and Lake Counties, delivering high‑quality nursing care in the comfort of their homes. This PRN role offers flexible scheduling while helping to prevent readmissions and improve patient outcomes.

Key responsibilities

• Perform comprehensive admission assessments, reassessments, and develop individualized care plans.
• Provide complex treatments, education, and pain management, coordinating with LPNs, Home Care Aides, families, and other disciplines.
• Monitor patient safety, identify risks of abuse or neglect, and escalate changes in condition to physicians.
• Maintain accurate, up‑to‑date documentation of patient status and interventions.

Required profile

• Graduate of an accredited nursing program with current Ohio RN licensure.
• Minimum one year of acute medical/surgical RN experience.
• Valid Ohio driver’s license and automobile insurance meeting $100,000/$300,000 coverage requirements.
• Availability for PRN shifts (5‑8 hour days between 9 am‑5 pm) with required holiday and on‑call coverage.

Required skills

• Basic Life Support (BLS) certification through AHA or Red Cross.
• Proficiency in patient assessment, care planning, and documentation.
