RN Homecare Case Manager

About the role

The RN Homecare Case Manager provides skilled nursing, wound care, and infusion services to patients recovering at home after hospital stays or procedures. Working independently in patients' homes across Cuyahoga County, you will coordinate care, educate patients, and ensure safe clinical outcomes.

Key responsibilities

• Perform peripheral and central line blood draws, IV insertions, and administer IV antibiotics and push medications.
• Provide comprehensive wound care, including measurement, documentation, and management of wound vacs.
• Assess patients in their home environment, develop care plans, and educate patients and families throughout the short‑term recovery process.
• Travel to patient homes within Cleveland, Old Brooklyn, Parma, Brook Park and surrounding areas, adhering to on‑call, weekend, and holiday schedules.

Required profile

• Graduate of an accredited nursing program with current Ohio RN licensure.
• Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and automobile insurance meeting specified coverage limits.
• At least one year of recent RN experience in an acute medical/surgical or home‑care setting.

Required skills

• Peripheral and central line blood draw techniques.
• Wound care management, including wound vac operation.
• IV insertion, medication administration, and central line handling.
