RN Home Health PRN

About the role

The Cleveland Clinic Health System is seeking a Registered Nurse for a PRN Home Health position. You will provide patient‑first care in the homes of patients in eastern Cuyahoga and Lake Counties, helping to prevent readmissions and ensure comfort.

Key responsibilities

• Provide direct nursing care in accordance with organization policies, procedures, and protocols.
• Perform admission assessments, reassessments, and ongoing patient monitoring.
• Develop, implement, and monitor individualized patient care plans.
• Supervise LPNs, Home Care Aides, and family caregivers.
• Assess safety risks, abuse, and neglect, and document pain using appropriate scales.
• Coordinate multidisciplinary care and escalate changes in condition to the physician.

Required profile

• Graduate from an accredited nursing program and hold current RN licensure.
• Minimum one year of clinical experience in an acute medical/surgical setting.
• Basic Life Support (BLS) certification through AHA or Red Cross.
• Valid Ohio driver’s license with proof of automobile insurance ($100,000/$300,000 coverage).

Required skills

• Basic Life Support (BLS) certification.
