About the role

The RN Home Health Hospice position is a PRN role within Cleveland Clinic Health System, providing skilled nursing care to patients in their homes across the Cleveland and Northeast Ohio area.

Key responsibilities

Provide direct nursing care according to organization policies, procedures, and protocols.
Implement and monitor patient care plans, conduct admission assessments and reassessments.
Supervise LPNs, Home Care Aides, and family caregivers.
Assess patient safety, abuse, neglect, and coordinate multidisciplinary care.
Document pain assessments, manage pain, and prioritize patient problems.

Required profile

Graduate from an accredited nursing program and hold current RN licensure in Ohio.
Minimum one year of clinical experience as an RN, preferably in an acute medical/surgical setting.
Basic Life Support (BLS) certification through AHA or Red Cross.
Valid Ohio driver’s license and proof of automobile insurance with required coverage.
