RN Home Case Manager – Summit and Cuyahoga Counties

About the role

The Cleveland Clinic Health System is seeking a Registered Nurse to provide high‑quality, patient‑first care in patients' homes across Summit and Cuyahoga Counties. You will work independently in the field and remotely to ensure safe transitions from acute care and prevent readmissions.

Key responsibilities

• Perform comprehensive admission assessments, reassessments, and ongoing nursing evaluations in patients’ homes.
• Deliver complex nursing treatments, education, and pain management while coordinating multidisciplinary care.
• Document care accurately, make follow‑up calls, attend virtual meetings, and arrange visit schedules.
• Identify safety risks, abuse, or neglect and act as the primary coordinator for each patient’s care plan.

Required profile

• Graduate from an accredited nursing program and hold a current Ohio RN license.
• Basic Life Support (BLS) certification through the American Heart Association.
• Valid Ohio driver’s license and proof of required automobile insurance.
• At least one year of recent RN experience in an acute medical/surgical or home‑care setting.
