RN Home Care Float – Flexible Home Nursing

About the role

The RN Home Care Float provides high‑quality nursing care to patients in their homes across eastern Cuyahoga, Lake and Geauga Counties. Working three 12‑hour shifts (10:00 am‑10:30 pm) with no weekends, holidays or on‑call duties, you will deliver assessments, complex treatments and education to help avoid readmissions.

Key responsibilities

• Perform direct nursing care, including visits on the uncovered list, late admissions and other patient visits as needed.
• Complete admission assessments, reassessments, and document pain, safety risks, and care plans.
• Coordinate multidisciplinary care, supervise starts of care and routine visits, and participate in quality‑improvement projects.
• Escalate changes in patient condition to the physician promptly.

Required profile

• Graduate of an accredited nursing program with current RN licensure.
• Basic Life Support certification (AHA or Red Cross).
• At least one year of clinical experience in an acute medical/surgical or home‑care setting.
• Valid driver’s license and proof of automobile insurance coverage (100k/300k).

Required skills

• Proficiency in mobile documentation and electronic health record systems.
• Ability to assess pain using standardized scales and manage pain appropriately.
• Strong clinical judgment to make independent decisions in the field.
