RN Healthcare Advocate (Remote)

About the role

Join Sailor Health’s mission-driven team to transform senior mental health care. This remote, flexible position allows you to work from home while supporting older adults on Medicare through compassionate advocacy.

Key responsibilities

• Build trusted relationships with older adults and their families, offering empathetic guidance.
• Navigate medical systems, social services, and emotional wellness needs, creating individualized care plans that address clinical needs and social determinants of health.
• Coordinate access to food, housing, transportation, medication management, appointments, and caregiver support.
• Collaborate with therapists, care coordinators, and the tech platform to deliver seamless, person‑centered care.
• Provide feedback to shape Sailor’s patient advocacy model as it scales.

Required profile

• Active Registered Nurse license in good standing.
• Experience in healthcare advocacy or care navigation with Medicare populations.
• Understanding of social determinants of health and their impact on outcomes.
• Strong clinical judgment, empathy, and problem‑solving abilities.
• Organized, communicative, and able to manage multiple patients.
• Based in the United States.

Required skills

• Comfort with learning new remote systems and technology platforms.
