RN Diabetes Patient Educator

About the role

The RN Diabetes Patient Educator joins Cleveland Clinic Weston Hospital to coordinate the diabetes education program, provide resources to physicians, caregivers and patients, and drive organizational improvements in diabetes care.

Key responsibilities

• Provide direct patient care using medical nutrition therapy and diabetes self‑management education.
• Collaborate with Endocrinology providers to design and administer nutrition and diabetes care plans.
• Complete comprehensive patient assessments and develop individualized care plans.
• Deliver education and counseling on diabetes and obesity management, using national guidelines and technology.
• Develop and contribute to education programs for clinicians and the public, and track outcomes.

Required profile

• Bachelor’s degree or higher in Nursing, Dietetics, Pharmacy or related field.
• Current Florida license to practice as RN, RD/RDN, or Pharmacist.
• Active certification as Registered Dietitian/Nutritionist (RD/RDN) and obtain CDCES certification within two years of hire.
• At least one year of experience as a Registered Dietitian, Registered Nurse, or Pharmacist, preferably in a diabetes program.

Required skills

• Proficiency with EPIC electronic health record software.
• Experience using Continuous Glucose Monitoring (CGM) devices.
• Ability to document patient encounters accurately in an electronic medical record system.
