About the role
The Per Diem RN Case Manager will join Cleveland Clinic Weston Hospital’s Utilization and Quality Assurance department, providing patient‑centered transitional care coordination during daytime shifts.
Key responsibilities
- Ensure compliance with accrediting bodies, state regulations, and CMS Conditions of Participation.
- Facilitate interprofessional communication and handoffs to maintain continuity of care.
- Assess barriers to patient transitions and develop individualized transitional care plans.
- Participate in patient rounding, huddles, and committee meetings.
Required profile
- Graduate of an accredited nursing program with current RN licensure.
- Basic Life Support (BLS) certification.
- Minimum three years recent clinical RN experience.
- Preferred BSN degree and certification in case management (CTM, ACM, CCM) within one year of eligibility.
- Experience in utilization review, quality improvement, or discharge planning is a plus.
