RN Care Coordinator – Orthopedics Trauma

About the role

The RN Care Coordinator in Orthopedics Trauma works within the Integrated Surgical Institute at Fairview Hospital. You will provide longitudinal care coordination and disease management for high‑risk orthopedic patients, aiming to improve outcomes and reduce readmissions.

Key responsibilities

• Collaborate with a multidisciplinary team to develop goals, plan interventions, and maximize patient outcomes.
• Identify patients with ongoing coordination needs and conduct targeted outreach.
• Perform comprehensive clinical assessments covering medical, behavioral, pharmacy, social, and end‑of‑life needs.
• Educate patients and families, monitor compliance with care plans, and update plans as needed.
• Serve as a liaison and advocate, managing transitions of care across settings and connecting patients to community resources.

Required profile

• Graduate of an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification.
• Three to five years of nursing experience.
• Cancer‑specific certification or ongoing qualifying education if working in oncology‑related areas.
