RN Care Coordinator – Gastroenterology

About the role

The RN Care Coordinator will work within the Gastroenterology department at Florida Weston Hospital to provide longitudinal coordination of care and disease management for patients with chronic gastrointestinal conditions.

Key responsibilities

• Collaborate with multidisciplinary teams to develop patient‑specific goals and interventions.
• Identify and outreach to patients requiring ongoing coordination, using registries and patient lists.
• Conduct comprehensive clinical assessments covering disease‑specific, medical, behavioral, pharmacy, social and end‑of‑life needs.
• Monitor patient adherence to treatment plans, provide education, coaching and reassess progress toward goals.
• Serve as liaison and advocate for patients and families, facilitating transitions of care and connecting to community resources.
• Ensure compliance with disease‑specific care pathways and support optimal utilization of APPs and other resources.

Required profile

• Graduate from an accredited nursing program with current RN licensure in the state.
• Three to five years of nursing experience; BLS certification required.
• Preferred: BSN, specialty certification, case‑management experience, PACU or Endoscopy experience, ASC experience.

Required skills

• Basic Life Support (BLS) certification.
• Registered Nurse (RN) licensure.
