Remote US Licensed Practical Nurse (LPN) – Inbound Care Management

About the role

We are seeking a Licensed Practical Nurse (LPN) to join our remote Chronic Care Management team in an inbound, call‑center‑style environment. The role involves answering patient calls, providing support for ongoing care management, and ensuring proper follow‑up and continuity of care.

Key responsibilities

• Answer inbound patient calls related to Chronic Care Management services.
• Assist patients with questions and concerns about their care plans.
• Review and follow established patient care plans during interactions.
• Identify and escalate patient needs or concerns according to protocols.
• Accurately document calls, updates, and follow‑up requirements.
• Meet KPIs for quality, productivity, and attendance while maintaining HIPAA compliance.

Required profile

• Active LPN license with a Compact State License (NY and CA required/accepted).
• Strong verbal and written communication, active listening, and customer service skills.
• Ability to handle multiple inbound calls in a fast‑paced environment.
• Basic computer proficiency and ability to complete a technology assessment.

Required skills

• Google Workspace (Gmail, Google Meet, G Suite).
• Desktop computer with dual‑monitor setup.
• Hard‑wired Ethernet connection (30‑foot cable provided).
