Remote Registered Nurse - Remote Patient Monitoring & Chronic Care Management

About the role

A full-time, remote Registered Nurse position focused on Remote Patient Monitoring and Chronic Care Management. The nurse coaches patients toward better health by helping them navigate, prevent and manage their health through continuous care programs, working with patients by phone or video.

Key responsibilities

• Welcome patients into continuous care programs and explain the benefits and services included.
• Educate patients on the frequency and use of their in-home monitoring devices.
• Create a personalized, comprehensive care plan with the patient via phone or video visits.
• Identify and address barriers to patient success and provide education and coaching on chronic conditions.
• Connect with patients regularly to review readings and give a monthly update of the plan of care.
• Serve as the first contact for non-emergent needs such as medication refills, scheduling and appointment reminders.
• Help patients navigate healthcare, including scheduling appointments, coordinating with specialists and completing preventative screenings.
• Review in-home device readings in real time during normal business hours and document trends and symptoms according to established protocols.
• Escalate concerning readings or symptoms to the provider and collaborate with virtual team members and in-office staff.

Required profile

• Active and unrestricted Georgia RN license (compact license preferred).
• At least three years of adult health experience preferred.
• Background in adult chronic conditions (medical surgical, outpatient/clinic, care management) preferred.
• Clean background check and drug screening.
• Comfortable working remotely but collaboratively.

Required skills

• Variety of electronic medical record (EMR) experience.
• Proficiency in basic computer skills.
• Problem-solving, multitasking, oral and written communication and teamwork skills.
