About the role

Join our Transitional Care Rehab Unit as a Licensed Practical Nurse (LPN) working 12‑hour shifts. You will provide direct patient care in a fast‑paced rehabilitation environment, supporting recovery and functional independence.

Required profile

Active Licensed Practical Nurse (LPN) license.
Experience in a rehab setting required.
Successful completion of post‑offer background check, physical and drug screening.
