Registered Nurse – Transition Team

About the role

Join BAYADA's Transition Team to provide complex home care for infants, children, and adults across Chester, Delaware, and Montgomery Counties. This full‑time position supports the hand‑off from hospital to home, offering a unique learning environment for rare and complex diseases.

Key responsibilities

• Deliver high‑quality nursing care to clients transitioning from hospital to home.
• Provide extensive training and education to the ongoing BAYADA care team.
• Travel throughout designated counties to meet client needs.

Required profile

• Current RN license with NICU, PICU, ICU, or home‑care trach/vent experience.
• Ability to commit to a consistent 40‑hour weekly schedule, including at least four weekend shifts per month.
• Willingness to work day or overnight shifts and cover weekends.

Required skills

• Proficiency in high‑tech adult and pediatric simulation assessments.
• Current CPR certification.
• Experience teaching and preparing other care team members.
