Occupational Therapist – Home Health Visits

About the role

BAYADA Home Health Care is seeking a full‑time Occupational Therapist to provide home health visits to adult and geriatric patients throughout Delaware County. You will join a multidisciplinary team delivering rehabilitative care that enables clients to remain safely at home.

Key responsibilities

• Conduct home visits in designated geographic territories and perform comprehensive functional evaluations.
• Assess cognitive, psychological, and motor performance, develop individualized intervention plans, and continuously revise care plans.
• Evaluate the home environment for hazards, recommend equipment and adaptations, and educate patients and families on safe, independent living.
• Document observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current PA Occupational Therapist license with at least one year of recent OT experience.
• Graduation from an accredited occupational therapy program.
• Ability to work independently, manage time effectively, and demonstrate strong interpersonal and problem‑solving skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
