LPN – Transition Team (Home Health Care)

About the role

BAYADA Home Health Care is seeking Licensed Practical Nurses to join its Transition Team, providing complex home care for infants, children, and adults across Chester, Delaware, and Montgomery Counties. The position is full‑time (40 hours/week) with day and overnight shift options.

Key responsibilities

• Facilitate the transition of patients from hospital to home care, ensuring continuity of complex care.
• Deliver high‑quality nursing care to clients with rare and complex diseases.
• Provide extensive training and education to the ongoing BAYADA care team.
• Travel throughout the designated counties to meet patient needs.

Required profile

• Experience in NICU, PICU, ICU, or home‑care trach/vent settings.
• Pediatric trach/vent experience is mandatory.
• Current CPR certification and ability to achieve passing scores on high‑tech adult and pediatric simulations.
• Commitment to a consistent 40‑hour weekly schedule, including at least four weekend shifts per month.

Required skills

• NICU, PICU, ICU nursing skills.
• Tracheostomy and ventilator management for both adults and pediatrics.
• High‑tech simulation proficiency.
• CPR certification.
