Registered Nurse – Start of Care

About the role

BAYADA Home Health Care is seeking a compassionate full‑time Registered Nurse to lead Start of Care and Oasis Admissions for adult and geriatric clients across Duval, Baker, Clay, and Nassau counties. The RN will provide in‑home assessments, medication administration, wound care, and case management to ensure safe, independent living for patients.

Key responsibilities

• Conduct home visits following a designated care plan and perform assessments, medication administration, wound care, and other treatments.
• Monitor client conditions, document observations on a tablet, and report changes to the Clinical or Client Services Manager.
• Execute and document physicians' orders, manage case coordination, and ensure accurate EMR entry.

Required profile

• Current Florida RN license with at least one year of recent clinical (medical/surgical) experience.
• Ability to work independently, manage time effectively, and travel to assigned cases.
• Strong interpersonal skills and solid computer proficiency.

Required skills

• Experience with electronic medical records (EMR) systems.
