Registered Nurse (RN) Home Care Admissions Nurse with $10,000 Sign‑On Bonus

About the role

The Lehigh Valley Health Network seeks a full‑time Registered Nurse to join its Home Care Admissions team. The RN will assess patients, determine appropriate levels of care, and develop individualized care plans while delivering patient‑centered education and support.

Key responsibilities

• Perform comprehensive initial and follow‑up assessments covering physical, psychological, and environmental factors.
• Collect and document CMS‑required data with high accuracy and formulate individualized plans of care.
• Communicate significant changes in patient status to physicians and the interdisciplinary team promptly.
• Provide education on end‑of‑life issues, palliative care, chronic disease management, pain control, and discharge planning.

Required profile

• Graduate of an NLN‑approved nursing program with a current RN license in Pennsylvania.
• At least one year of recent direct‑care experience, preferably in home health or hospice.
• Ability to travel to patient assignments and lift/transfer patients as required.
• Current Basic Life Support (BLS) certification.

Required skills

• Proficiency with electronic medical record (EMR) platforms and multiple computer applications.
• Knowledge of CMS data collection, Conditions of Participation, and regulatory documentation.
