Registered Nurse RN Care Manager - Hospice

About the role

UChicago Medicine AdventHealth is hiring a full-time day-shift Registered Nurse Care Manager for its hospice program in the Great Lakes region. The schedule is Monday to Friday, 8am to 5pm, with weekend coverage every 8 to 10 weeks. The location covers the South Suburb area including LaGrange, Hinsdale, Western Springs, Lemont, Bolingbrook, Romeoville, Downers Grove and Westchester.

Key responsibilities

• Develop and maintain positive working relationships within the practice and other departments.
• Implement and monitor patient care plans to ensure effective treatment.
• Monitor, record and communicate patient condition, and act as the primary coordinator of all disciplines for coordinated patient care.
• Note and carry out physician and nursing orders accurately and promptly.
• Assess and coordinate discharge planning with the healthcare team.
• Lead and mentor staff to ensure optimal performance and competency in care delivery.
• Conduct holistic, individualized assessments of each patient, considering age, physiological and psychological status.
• Administer commonly used drugs and solutions in Cardiology and IR procedures.
• Document all phases of patient status while responsible for their care.
• Participate in transferring patient information to ensure continued quality care and maintain a safe, clean working environment.

Required profile

• Registered Nurse (RN) license required.
• Associate's degree in Nursing required; Bachelor's degree preferred.
• 1+ years of experience preferred.
• Basic Life Support (CPR) certification required.
• Valid driver's license and auto insurance (Illinois) required.

Pay range: $70,595.20 - $118,726.40.
