Registered Nurse – Remote Patient Monitoring (Abbeville)

About the role

The Registered Nurse (RN) will provide nursing support for chronic‑care and wellness patients in child‑bearing years across mobile and fixed BLOOM clinics in Abbeville and surrounding counties. The role combines in‑person care, telehealth, and remote patient monitoring (RPM) to improve patient outcomes.

Key responsibilities

• Support the CRH nurse practitioner with patient education, work‑ups, blood draws, surveys, and clinical documentation.
• Serve as the first point of contact for RPM enrollees, conduct outreach, obtain consent, and schedule enrollment appointments both in‑person and virtually.
• Develop and manage individualized RPM care plans, monitor patient data, and communicate abnormal readings to providers.
• Maintain inventory of RPM devices, order supplies, and pre‑register equipment for mobile events.
• Assist clinic staff and principal investigators with the implementation of approved research protocols.

Required profile

• Active RN license with experience in chronic‑care, maternal health, or wellness populations.
• Strong verbal and written communication skills for patient education and provider collaboration.
• Ability to work across multiple platforms, manage inventory, and support clinical research.

Required skills

• Proficiency with Electronic Health Record (EHR) systems.
