Registered Nurse – Mental Health & Community Care

About the role

This full‑time Registered Nurse position focuses on delivering direct mental‑health services in the community. You will join an ACT (Assertive Community Treatment) team, providing crisis intervention, medication management, and health assessments to individuals with serious mental illness and co‑occurring substance use.

Key responsibilities

• Participate in daily morning team meetings to coordinate care.
• Manage and administer oral and injectable medications, including IMs, and ensure accurate documentation in Medication Administration Records.
• Maintain the monthly IM calendar, log medication deliveries, and complete toxicology request forms.
• Monitor vital signs and medication side effects, reporting findings to the clinical team.
• Conduct annual nursing assessments, daily progress notes, and refer patients to community physicians as needed.
• Provide staff education on medication policies, secure record storage, and coordination with prescribers and pharmacies.
• Perform additional duties aligned with program goals as directed by the Team Leader.

Required profile

• Current New York State Registered Nurse license.
• Associate’s, Bachelor’s or Master’s degree in nursing.
• Clinical experience serving mentally ill or chemically‑addicted populations.

Required skills

• Accurate record‑keeping and transcription of orders.
• Monitoring of vital signs and medication side effects.
• Ability to work independently in community settings and collaborate within a multidisciplinary team.
