Registered Nurse - Home Visit

About the role

Almost Family, a part of LHC Group and the Optum family, is hiring a Registered Nurse in Fond du Lac, Wisconsin. The nurse provides and directs nursing care to patients in their homes as prescribed by the physician and in line with laws, regulations and agency policies, while coordinating the overall plan of care with other health care professionals.

Key responsibilities

• Provide high-quality clinical services within scope of practice and infection control standards
• Coordinate care with the patient's care team from admission to discharge
• Complete clinical nursing assessments per federal, state program requirements and payer needs
• Verify patient eligibility and medical necessity as defined by payer and agency policy
• Develop and revise individualized plans of care with other community providers, ensuring frequency and duration meet patient needs

Required profile

• Current and unrestricted RN licensure in the state of practice
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation
• Ability to work in any home situation regardless of the client's background
• Preferred: 1+ years of RN experience; ability to work independently

• Good communication, writing and organizational skills

What we offer

• Hourly pay of $35.85 to $53.75 for full-time employment, depending on factors such as experience and certifications
