Registered Nurse – Home Health (Weekends Admissions)

Key responsibilities

• Perform home visits, follow designated care plans, and administer medication, wound care, and treatments.
• Monitor client conditions, document observations on a tablet, and report changes to Clinical or Client Services Manager.
• Execute doctors' orders, conduct client assessments, and coordinate case management.

Required profile

• Current Vermont RN license and at least one year of recent medical/surgical experience.
• Ability to work independently, manage time effectively, and travel to assigned cases.
• Strong interpersonal skills and solid computer proficiency.

Required skills

• Experience with electronic medical records (EMR) systems.
• Proficiency using a tablet for documentation.
