Registered Nurse – Home Health Care (Vermont)

About the role

Join BAYADA Home Health Care as a Registered Nurse delivering skilled nursing and rehabilitative care to clients in their homes across Vermont. You will work independently within a multidisciplinary team, ensuring high‑quality, compassionate care that enables patients to remain at home.

Key responsibilities

• Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
• Provide acute and post‑acute care including CHF, COPD, wound care, IV therapy, and post‑surgical support.
• Tri­age urgent clinical situations and make real‑time decisions in the home setting.
• Perform medication reconciliation, patient education, and safety monitoring.
• Document care accurately using point‑of‑care technology.

Required profile

• Current RN license in good standing in Vermont.
• Graduated from an accredited nursing program.
• At least one year of recent hands‑on experience with adult/geriatric patients in acute, emergency, home health, hospice, ICU, med‑surg, sub‑acute, or rehab settings.
• Ability to work independently and escalate concerns when needed.
