Registered Nurse – Home Health Visits (Med Surg & Wound Care)

About the role

BAYADA Home Health Care is seeking a Registered Nurse to join its home health visit team serving Cuyahoga County. You will provide skilled nursing and rehabilitative care in patients' homes, enabling them to stay safely at home.

Key responsibilities

• Perform comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
• Manage acute and post‑acute care needs such as CHF, COPD, wound care, IV therapy, and post‑surgical care.
• Triage and handle urgent clinical situations independently in the home setting.
• Conduct medication reconciliation, patient education, and safety monitoring.
• Document wound assessments, care plans, and point‑of‑care notes efficiently.

Required profile

• Current Ohio nursing license in good standing.
• Graduated from an accredited nursing program.
• At least one year of recent hands‑on medical experience with adult/geriatric patients in acute, med‑surg, or related settings.
• Ability to work independently, prioritize urgent visits, and adapt to varied home environments.
