Registered Nurse Home Care Admissions – Weekend with $15,000 Sign‑On Bonus

About the role

The Registered Nurse (RN) Home Care Admissions Nurse works weekends to assess patients, determine appropriate levels of care, and develop individualized care plans in collaboration with physicians and interdisciplinary teams.

Key responsibilities

• Perform comprehensive initial and follow‑up assessments covering physical, psychological, and environmental factors.
• Collect and document data per CMS regulations with high accuracy.
• Develop individualized plans of care based on physician orders and current scientific findings.
• Communicate significant changes in patient status promptly to physicians and care team members.
• Provide patient, family, and staff education on end‑of‑life issues, palliative care, chronic disease management, pain and symptom control, home care, hospice, and discharge planning.

Required profile

• Graduate of an NLN‑approved nursing program.
• At least 1 year of recent direct patient care experience.
• RN licensure in Pennsylvania (or NJ) upon hire.
• Basic Life Support (BLS) certification from the American Heart Association.
• Valid driver’s license for patient travel.

Required skills

• Proficiency in patient education techniques.
• Ability to complete regulatory data set collection accurately.
• Experience navigating multiple computer applications and EMR platforms.
• Strong verbal and non‑verbal communication skills.
