Registered Nurse – Diabetes Education Program

About the role

The Registered Nurse I supports the Diabetes Education Program, providing education, care instructions, and assessments for patients. The role involves both office and field work, including community outreach and school training.

Key responsibilities

• Develop and maintain individualized diabetes self‑education and support plans, monitoring progress quarterly.
• Perform comprehensive initial and quarterly reassessments, collecting biometric data such as blood pressure, glucose, and HbA1c.
• Collaborate with referring providers and other healthcare professionals regarding education plans and outcomes.
• Provide preventive education to the community, schools, and students.
• Document and maintain education records, state health department records, and travel to various education locations.

Required profile

• Successful completion of a Registered Nurse education program.
• Licensed RN in Texas, CPR certified, and a valid Texas driver’s license.
• Ability to work beyond normal office hours and on weekends if needed.
• Strong written, communication, and interpersonal skills.

Required skills

• Use and management of electronic medical/health records systems.
• Operation of biometric equipment for assessments.
• Knowledge of standard nursing procedures and confidentiality laws.
