Registered Nurse Case Manager – Home Health Visits

About the role

BAYADA Home Health Care is seeking a full‑time Registered Nurse Case Manager to provide skilled nursing and rehabilitative care to clients in Montgomery County, Maryland. You will work independently in patients' homes, delivering high‑quality, compassionate care.

Key responsibilities

• Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
• Manage acute and post‑acute conditions such as CHF, COPD, wound care, IV therapy, and post‑surgical care.
• Triage and handle urgent clinical situations independently in the home setting.
• Perform medication reconciliation, patient education, and safety monitoring.
• Document care accurately using point‑of‑care technology.

Required profile

• Current Maryland RN license in good standing.
• One year of recent hands‑on experience with adult/geriatric patients in acute, home health, hospice, ICU, med‑surg, sub‑acute, or rehab settings.
• Ability to work independently, prioritize urgent visits, and escalate concerns when needed.
• Strong problem‑solving skills and cultural sensitivity.
