Registered Nurse Case Manager - Home Health

About the role

Amedisys, part of the Optum family, offers home health, hospice and community-based care. As a Nurse Case Manager, you will guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning and managing ongoing outpatient care, under the supervision of an RN or MD. The position is advertised with a $15K sign-on bonus.

Key responsibilities

• Perform patient assessments and help the care team develop and implement a plan of care
• Make referrals to other disciplines or document the rationale for not doing so
• Promote patient health and independence through teaching and rehabilitative measures, helping patients learn self-care techniques
• Supervise LPNs and HHAs
• Complete documentation timely and accurately, and share patient progress with the clinical manager and care team
• Plan and deliver staff education, perform on-call duties and take part in clinical development and continuing education

Required profile

• Current RN license for the state(s) of assignment
• 1+ year of clinical experience as a Registered Nurse
• Valid driver's license, reliable transportation and liability insurance
• Current CPR certification

What we offer

• Annual total cash target pay of $67,905 - $101,857, with per-visit point pay and an hourly rate for non-visit activities
