Registered Dietitian

About the role

Habitat Health is seeking a dynamic, service‑oriented Registered Dietitian to join its multidisciplinary care team. You will work closely with primary care providers and other clinicians to deliver whole‑person, integrated nutrition care for older adults enrolled in the PACE program.

Key responsibilities

• Conduct comprehensive nutritional assessments and reassessments for participants.
• Develop individualized nutrition care plans that consider medical needs, social circumstances, and food access.
• Provide nutrition education to participants, caregivers, and staff, including home‑visit instruction.
• Order and monitor oral nutritional supplements as needed.
• Coordinate referrals for meal preparation, grocery shopping, and other food‑security services.
• Implement medical nutrition therapy for high‑risk participants and track dietary compliance.

Required profile

• Registered Dietitian credential (RD) and active licensure.
• Passion for supporting elderly populations and experience in community‑based or geriatric nutrition.
• Ability to work collaboratively within an interdisciplinary team.
• Strong communication skills for education and counseling.

Required skills

• Conducting nutritional assessments and developing care plans.

• Knowledge of dietary modifications for chronic conditions.
