Overdose Prevention - Outreach Care Coordinator

About the role

The Outreach Care Coordinator provides client‑centered, trauma‑informed street outreach to people experiencing homelessness in Clark County. The primary focus is injury prevention and drug checking, while also supporting housing navigation and care coordination.

Key responsibilities

• Provide comprehensive injury prevention services through mobile outreach, distributing safer‑use supplies, naloxone, and wound‑care materials.
• Conduct residue drug checking: collect samples, perform testing, communicate results, and educate participants on overdose risk reduction.
• Maintain documentation, inventory, and quality‑assurance procedures for drug‑checking supplies and reporting to funding sources.
• Assist participants in linking to health, mental‑health, substance‑use, and housing services, and record interactions in the HMIS database.
• Respond to crisis situations, attend required trainings, and support program operations during staff absences.

Required profile

• At least two years of experience working with people experiencing homelessness, substance‑use disorder, or related health challenges.
• Knowledge of injury‑prevention, overdose‑prevention, and infectious‑disease prevention practices.
• Ability to provide non‑judgmental, culturally responsive services and safely handle biohazardous materials.

Required skills

• Proficiency with Microsoft Office Suite.
• Experience or willingness to be trained in residue drug‑checking procedures.
• Ability to collect, maintain, and report program data for multiple contracts and funding sources.
