Psychiatric Mental Health Nurse Practitioner

About the role

We are seeking a licensed Psychiatric Mental Health Nurse Practitioner to join our Los Altos clinic. You will work within an interdisciplinary team to deliver high‑quality, coordinated care for adults living with severe mental illness.

Key responsibilities

• Provide comprehensive behavioral health management, monitoring patients through various phases of recovery.
• Conduct psychiatric evaluations and assessments for individuals with schizophrenia, schizoaffective disorder, bipolar I, and treatment‑resistant depression.
• Manage medication regimens, adjust treatments, and ensure safety and efficacy.
• Develop and update individualized treatment plans that integrate therapy, primary‑care, nutrition, and community resources.
• Collaborate daily with psychiatrists, therapists, primary‑care providers, dietitians, and community health workers.

Required profile

• Current licensure as a Psychiatric Mental Health Nurse Practitioner (PMHNP) in California.
• Experience providing care to patients with severe mental illness.
• Ability to work in a high‑acuity, team‑based outpatient setting.
• Commitment to continuous learning and evidence‑based practice.
