About the role
Using advanced practice nursing skills, you will assess and coordinate care for a diverse group of patients in general medicine. You will collaborate with a multidisciplinary team, patients, and families to ensure safe, high‑quality, and cost‑effective care.
Key responsibilities
- Develop and implement individualized care plans using evidence‑based risk‑stratification tools.
- Perform utilization review and ensure delivery of concurrent and post‑hospital care.
- Drive performance‑improvement initiatives and support discharge planning.
Required profile
- BSN or MSN (ASN accepted for current UCLA Health nursing staff).
- CA RN license and BLS certification.
- Recent experience in case management, utilization management, and discharge planning.
- Minimum three years of acute hospital experience or equivalent education and experience.
- Strong leadership, communication, and analytical skills.
