PRN RN Case Manager – Hillcrest Hospital

About the role

The PRN RN Case Manager provides transitional care coordination for patients across the continuum of care at Cleveland Clinic Hillcrest Hospital. This per‑diem position works primarily day shifts, with occasional weekend and holiday coverage, supporting patient advocacy, holistic assessment, and care planning.

Key responsibilities

• Collaborate with inter‑professional teams to ensure effective transitional care and compliance with accrediting standards.
• Develop individualized transitional care plans with patients and their care partners.
• Conduct patient rounding, huddles, and committee meetings to promote continuity of care.
• Identify and mitigate barriers to successful patient transitions, completing thorough handoffs to the next level of care.

Required profile

• Graduate of an accredited nursing program (Diploma, ADN, or BSN) with current state RN licensure.
• Minimum three years recent RN experience; preferably with case management, utilization review, or quality care management.
• Basic Life Support (BLS) certification.
• Ability to work full range of motion, stand, walk, and perform sedentary tasks as required.

• Case management certification (CTM, ACM, CCM, or AHA) within one year of eligibility preferred.
