PRN Physical Therapist – Homecare (South Territory)

About the role

The Cleveland Clinic Business Operations Center is seeking an experienced PRN Physical Therapist to provide home‑based care across Medina, Stark, Wayne, Summit and Portage Counties. You will deliver orthopedic, Parkinson’s, COPD and Multiple Sclerosis therapy directly in patients’ homes.

Key responsibilities

• Conduct initial and ongoing assessments and develop individualized plans of care.
• Provide therapy interventions using standard physical therapy techniques appropriate to the patient’s age and condition.
• Supervise physical therapy assistants, students and other support personnel.
• Maintain complete, up‑to‑date documentation of patient condition, treatment effectiveness and timeliness.
• Participate in interdisciplinary assessments and hospice team meetings as needed.

Required profile

• Bachelor’s degree or certificate from an APTA‑accredited Physical Therapy program.
• Current Ohio PT licensure and Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and proof of automobile insurance meeting policy requirements.
• One year of clinical experience in an acute, rehab or skilled nursing setting (preferred).

Required skills
