PRN Home Health RN – West Cuyahoga County

About the role

The PRN Home Health RN provides skilled nursing care to patients in their homes within the West of I-77 service area. This position offers a flexible schedule that supports work‑life balance while delivering high‑quality, patient‑first care.

Key responsibilities

• Perform comprehensive admission assessments, reassessments, and ongoing patient evaluations.
• Develop, implement, and monitor individualized care plans and coordinate multidisciplinary services.
• Provide direct nursing care, complex treatments, and education to prevent readmissions.
• Supervise LPNs, Home Care Aides, and family caregivers.
• Document pain assessments, manage pain, and report changes in condition to physicians.

Required profile

• Graduate of an accredited nursing program with current Ohio RN licensure.
• Minimum one year of acute medical/surgical RN experience and six months of home‑care nursing.
• Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and automobile insurance meeting required coverage.
• Ability to work varying shifts, including weekends and holidays, with a 48‑hour/ six‑week schedule.

Required skills

• Basic Life Support (BLS) certification.
• Infusion therapy experience (preferred).
• Proficiency with mobile documentation and technology‑driven home‑care tools.
