Physical Therapist – Senior Living Visits

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to provide home health visits for adult and geriatric clients in senior living communities throughout the Sinking Spring area. This full‑time position offers a $5,000 sign‑on bonus and the opportunity to work within a multidisciplinary team delivering rehabilitative care.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests, mobility assessments, balance testing, and safety evaluations.
• Develop and implement individualized care plans, including therapeutic exercises, gait training, and wound‑care interventions.
• Educate clients and families on rehabilitative activities to promote independent living.
• Document all observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current Pennsylvania Physical Therapist license.
• Minimum one year of recent clinical experience as a licensed PT.
• Graduation from an APTA‑approved PT program.
• Ability to work independently, manage time effectively, and solve problems.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Strong computer skills for tablet‑based documentation.
