Physical Therapist – Senior Living Outpatient Visits

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to provide outpatient home‑health visits for senior‑living clients in the Sinking Spring area. The role can be part‑time or full‑time and involves traveling to client homes to support independent living.

Key responsibilities

• Make scheduled outpatient visits within designated geographic territories.
• Perform diagnostic tests such as range‑of‑motion, strength, balance, gait, and safety assessments.
• Implement wound‑prevention and wound‑care interventions using evidence‑based dressings.
• Develop and adjust individualized care plans that include therapeutic exercises, gait training, and balance work.
• Collaborate with nursing and other disciplines to integrate the PT plan into the overall care plan.
• Educate clients and families on rehabilitative activities to promote health and independence.
• Document all observations, interventions, and evaluations on a tablet‑based EMR system.

Required profile

• Current state Physical Therapist license.
• At least one year of recent PT experience.
• Graduation from an APTA‑approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and problem‑solving abilities.

Required skills

• Proficiency with electronic medical record (EMR) systems.
