Physical Therapist – Rehab Services Schuylkill

About the role

Lehigh Valley Health Network seeks a licensed Physical Therapist to join its Rehab Services team at the Schuylkill location. The role focuses on evaluating, planning, and delivering physical therapy to restore function and improve quality of life for patients receiving home care.

Key responsibilities

• Develop and implement individualized physical therapy plans of care based on patient assessments and physician diagnoses.
• Provide evidence‑based treatments including therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.
• Perform reassessments, modify treatment plans as needed, and document progress according to regulatory and billing guidelines.
• Communicate patient status and limitations to supervisors, physicians, and interdisciplinary team members.

Required profile

• Bachelor’s Degree in Physical Therapy (Master’s or Doctorate preferred).
• Licensed Physical Therapist in the State of Pennsylvania (license required upon hire).
• American Heart Association Basic Life Support certification (required upon hire).
• Ability to travel to patient homes and perform physical tasks such as lifting, pushing, and walking up to 8‑10 miles per shift.
