About the role

Lehigh Valley Health Network seeks a per‑diem Physical Therapist to provide home‑care services, restoring function and independence for patients in the East Stroudsburg area.

Key responsibilities

  • Evaluate, plan, and administer physical therapy programs to restore function and prevent disability.
  • Implement individualized treatment plans using therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.
  • Perform reassessments, modify care plans, and document progress according to regulatory standards.
  • Communicate patient status and limitations to supervisors, physicians, and team members.

Required profile

  • Bachelor’s Degree in Physical Therapy.
  • Licensed Physical Therapist in the State of Pennsylvania.
  • American Heart Association Basic Life Support certification (Pennsylvania).
  • Ability to travel to patient homes and lift/carry 40‑50 lb.
