Physical Therapist – Home Health (Per Diem)

About the role

BAYADA Home Health Care is seeking a Per Diem Physical Therapist to provide home health visits for clients in Horry County, SC. You will join a multidisciplinary team delivering rehabilitative care that enables patients to remain safely at home.

Key responsibilities

• Make home visits to clients within designated geographic territories.
• Perform diagnostic tests, range‑of‑motion measurements, strength, balance, and safety assessments.
• Implement wound‑prevention and wound‑repair interventions using evidence‑based dressings.
• Develop and adjust individualized care plans, including therapeutic exercises, gait training, and balance work.
• Educate clients and families on rehabilitative care, safety, and independent living.
• Document all observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current South Carolina Physical Therapist license.
• Minimum one year of recent PT experience.
• Graduated from an APTA‑approved PT program.
• Ability to work independently, manage time effectively, and solve problems.
• Strong interpersonal and organizational skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills, including tablet documentation.
