Physical Therapist - Home Health

About the role

Optum, a clinician-led care organization within UnitedHealth Group, is hiring a Physical Therapist Home Health in Las Vegas, NV. As part of the Optum Care Delivery team, you will deliver physical therapy services to patients in their homes, working alongside colleagues in a collaborative environment.

Key responsibilities

• Provide physical therapy services according to the physician's plan of care, including assessment of therapeutic, rehabilitative and functional status and evaluation of the home environment
• Initiate the physical therapy program and instruct caregivers and family members in the phases of care and the program goals, taking part in case conferences
• Prepare and submit clinical and progress summaries on time
• Take part in discharge planning
• Supervise physical therapy assistants per organization policy and state regulations, and re-evaluate patients at least every 7 days of treatment or every 21 days, whichever comes first
• Maintain contact with other care providers to ensure coordinated care and keep clinical competency up to date
• Work agency on-call coverage as outlined in agency policy

Required profile

• Graduate degree from an accredited physical therapy program
• Active, unrestricted Nevada Physical Therapist license
• CPR certification, or ability to obtain it within 30 days of hire
• 2+ years of experience as a Physical Therapist
• Driver's license and access to reliable transportation to reach patient sites within the designated area
• Preferred: 1+ year of home health experience, weekend availability

Required skills

• Preferred: experience with electronic medical records systems
• Preferred: experience with OASIS documentation

• Salary range of $72,800 to $130,000 annually for full-time employment
