Home Health and Hospice Licensed Practical Nurse

About the role

An established agency is looking for compassionate, skilled and experienced Licensed Practical Nurses in the Las Vegas area to provide patient-centered care in patients' homes. You will work with an interdisciplinary team focused on dignity, comfort and holistic care for home health and end-of-life patients. Bi-lingual and Spanish-speaking candidates are encouraged to apply.

Key responsibilities

• Provide skilled nursing care to home health and hospice patients
• Monitor patient conditions and report changes to the RN/Clinical Supervisor
• Administer medications and treatments as ordered
• Perform wound care, catheter care and other skilled nursing interventions
• Educate patients and families on care plans, disease processes and comfort measures
• Document all patient care accurately
• Collaborate with the interdisciplinary team to ensure continuity of care

Required profile

• Current Nevada nursing license in good standing
• At least 1 year of nursing experience; home health or hospice experience preferred
• High school graduate or GED
• Reliable transportation and a valid driver's license
• Able to travel within the service area
• Willingness to pass a drug screen and clear background check
